I showed the patient the door, told him to scarper, and said that he was a malingering valetudinarian and a confounded pest; to which on this occasion he could reply only with a vehemently extended digit for, following my impulsive frenectomy of his undoubted powers of iotrogrenic invective, any further attempts at sarcasm had been reduced to this obscene but thankfully mute flourish.
