As the board sat somberly staring at an Ishikawa diagram that analyzed the car manufacturer's recent woes, one of the executives got up to to comment on the causes: "The leakage of hypnagogic gas inside the vehicle, and the engineers' omission of the brake-fluid container's tare, are less shocking to me than the leakage of the holes in our quality control to the likes of Fleet Street".
